2026–2027 Health Requirements for the School Year

As summer approaches, we would like to remind all FISD families of the New York State health requirements that must be completed within 14 DAYS of the start of the 2026–27 school year. Please plan accordingly when scheduling your child's medical appointments this summer.

Each student enrolled at FISD must have a satisfactory health examination performed by a New York State- licensed physician or Nurse Practitioner (NP) within 12 months prior to the start of the school year. Health examinations are required for students entering:

  • Pre-Kindergarten or Kindergarten

  • 1st Grade

  • 3rd Grade

  • 5th Grade

  • Any grade level when entering FISD for the first time

New York State immunization requirements for 2026-2027, remain unchanged from the 2025–26 school year. Students who are at least 10 years old and entering the 6th grade, are required to have 1 DOSE of the Tdap vaccine. These requirements are based on scientific and public health guidance and are designed to protect children, families, and communities from preventable diseases.

A Dental Health Certificate is requested from each student at the same time that health certificates are required.

In addition, if your child has ASTHMA and requires rescue medication, an ALLERGY to FOOD, BEE bites or SEIZURES, your doctor MUST fill out an Emergency Care Plan (ECP) form, that is on our school website. Allergies to medicine, not given at school, should be noted on their Annual Health Exam form.

NYS now REQUIRES an Over the Counter Medication Permission form to be filled out by the parent or guardian and your child’s doctor. These are things such as Caladryl Clear lotion for mosquito bites and mild rashes, Vaseline to dry lips and Vanicream/Bag Balm lotion for dry skin. This form is on our school website. PLEASE RETURN THIS FORM TO SCHOOL AS SOON AS POSSIBLE.

All students will receive annual Hearing, Near and Far Vision testing, unless their own pediatrician’s office screened them or they have a private eye doctor. Color Perception is done once as a new arrival.

Heights, weights and blood pressures are done annually on all students.
